CONNECTION ORIENTED PATIENT-PROVIDER EXCHANGE IN PRIMARY CARE

Connection Oriented Patient-Provider Exchange in Primary Care (COPE_PC) is a platform where family physicians and patients can access information and resources to address social isolation in older adults with multimorbidity. Social isolation is an increasing problem for older adults with multimorbidity, and can lead to a number of health problems. This COPE_PC platform provides an opportunity for family physicians to work with their patients to identify and potentially lessen social isolation.
