This section of the Family Physician Education Module contains five vignettes describing older adults with multimorbidity that have presented to their family physicians for care. These vignettes where developed using existing research evidence, along with findings from the Untold Story qualitative interviews and questionnaire completed by patients. We are asking you to go through these five vignette simulations to practice using the COPE_PC Tool.

  1. Click on a vignette found in the left-side navigation bar of this webpage.

  2. Read the vignette.

  3. After reading the vignette, use your clinical expertise to reflect on what part of the patient’s situation resulted in them experiencing social isolation.

  4. When you have finished your reflection, please click on the REVEAL button. This button reveals colour coded phrases to share the research evidence suggesting why patient might be experiencing social isolation and their family physician’s perspective.

  5. Consider if what you see in the REVEAL supports your clinical assessment? Did you identify other factors? Did the REVEAL suggest new factors you had not considered? Did the family physician’s response resonate with you?

  6. Click on the COPE_PC TOOL SIMULATION button. This button provides the COPE_PC Tool questions discuss with the patient to facilitate a discussion about social isolation.
